CD Rams Sports Physicals
CDSD is partnering with Penn State Health Sports Medicine to offer free PIAA Pre-Participation Physicals for the 2026-27 school year.
Dates & Location
- Thursday, June 4, 2026: 3-6pm in CDHS Main Gymnasium
- Wednesday, July 29, 2026: 3-6pm the CDHS Main Gymnasium

- CDSD students in grades 7–12 for the 2026–2027 school year
- Students trying out for a CDSD school-sponsored PIAA sport
Sign Up For Free Sports Physical
- Select one of the Rams Sign-up Links below.
- Enter Student Athlete's name using parent or guardian email.
- Sign up only one student per time slot.
- No sign-ups after June 3rd for June 4th Physicals.
- No sign-ups after July 28th for July 29th Physicals.
- Walk-ins may be allowed but are not guaranteed.
Important Reminders
- To get a free physical: All students must bring a completed PIAA Section 5 and a blank PIAA Section 6 to their appointment.
- PIAA Section 5: Must be electronically completed and signed by a parent/guardian in Healthy Roster, printed and brought to Sports Physicals.
- PIAA Section 6: Blank forms can be printed using the link below or from the Healthy Roster Resource Library.
- PIAA Forms completed during the school sponsored exam will be returned to the Student Athlete for the parent/guardian to upload to Healthy Roster.
- PIAA 2026-27 Pre-participation paperwork must be dated on or after May 1, 2026.
- CDSD free Sports Physicals are valid for the entire 2026-2027 school year for all Fall, Winter, and Spring Sports.

NEW ATHLETES: Sports Pre-Participation Requirements
All CDSD and PIAA Sports Pre-Participation Paperwork must be submitted online through Healthy Roster. Paper documents will no longer be accepted.
Healthy Roster is a secure and private electronic medical records system that is HIPAA and FERPA compliant. Parents/Guardians of any student interested in trying out for a CDSD team must have a Healthy Roster Account. Healthy Roster requires a one-time free registration for Parents/Guardians. Accounts will stay active until the student graduates. Upon graduation, the account will automatically become inactive.
CDSD Athletic Trainers will use Healthy Roster to keep track of sports pre-participation paperwork, medical records, injuries, and doctor’s notes. Healthy Roster will make it easier and faster for Parents/Guardians to complete the CDSD and PIAA pre-participation forms online and for CDSD to make sure all athletes are cleared to participate.
All pre-participation paperwork must be completed and signed on or after May 1st per PIAA guidelines for the 2026-27 school year. Information cannot be edited once it has been submitted. Please double-check all information before moving to the next section. If you notice a mistake, contact Lisa Wawrzyniak, Central Dauphin Athletic Trainer, at lwawrzyniak@cdschools.org to have the document deleted. The document will then need to be completed and submitted again.
SPORTS PRE-PARTICIPATION REQUIREMENTS
STEP 1. Create a New Athlete Profile (NEW ATHLETES ONLY)
- Complete the CD Rams New Athlete Profile Form.
- Already have a Healthy Roster Account? Do NOT submit the CD Rams New Athlete Profile Form if your student already has a Healthy Roster Account from a previous school year. You can log in to that pre-existing account by clicking this link: Healthy Roster Login Dashboard.
- If you are a parent/guardian of more than one New Athlete, complete & submit this form for each New Athlete using the same email address.
STEP 2. Create a Healthy Roster Parent/Guardian Account (NEW ATHLETES ONLY)
- Initial Account Setup Tip: Computer often works better than the Healthy Roster App!
- Email Invitation: Healthy Roster will send an email invitation from noreply@healthyroster.com to the Parent/Guardian email listed on the New Athlete Profile form within one week from the date the form is submitted.
- Where is my Healthy Roster Email Invitation? Check your Spam/Junk mail folder for the Healthy Roster Invitation Email. If you use a CDSD email, your invitation may go to Barracuda. If you do not receive an email invitation from Healthy Roster within one week of submitting this form, please contact Lisa Wawrzyniak, Central Dauphin Athletic Trainer, at lwawrzyniak@cdschools.org.
- Open your email with subject: “Join Central Dauphin School District on Healthy Roster.”
- Sign up via Website: Click on the link in this email that states: “Click here to sign up via the website instead.”
- Unique Access Code: Enter the 6-digit Unique Access Code (different for each athlete) found in the Healthy Roster email invitation.
- Create New Account: Click “Next” and then select: “I need to create a new account.”
- Already have an account and are adding a new athlete? Use the same email address as your existing account. Your new athlete will be added automatically to your account. In the app, swipe left or right to switch between athletes.
- Follow the steps on the screen to finish setting up your account.
- Download the Healthy Roster App: Download the app from your phone’s App Store. Log in with the username and password you created for the web version. Use the authorization code sent to your email to finalize the app set up.
STEP 3. Complete all Healthy Roster Missing Documents (ALL ATHLETES)
- When prompted to Add Document, select the "Fill Online" box.
- PIAA Section 6 is the only document in Healthy Roster that cannot be completed electronically.
- Rejected Documents: Submitted documents are reviewed and approved/rejected by the CD Athletic Trainers. If a form is rejected, it will show again under “Missing Documents.” An email from no-reply@notifications.healthyroster.com explaining why the document was rejected and what needs to be fixed will be sent to you. Make the requested corrections and resubmit the document for review. Athletes are not cleared until all required documents have been approved.
STEP 4. Obtain Physical by an Authorized Medical Examiner on PIAA Sec 6 form dated May 1st or after (ALL ATHLETES)
- PRINT: completed PIAA Section 5 (complete electronically first on Healthy Roster)
- PRINT: blank PIAA Section 6
- Take completed PIAA Section 5 Health History & Blank PIAA Section 6 to Physical Appointment
- Obtain Physical by Authorized Medical Examiner (MD, DO, PAC, CRNP, or SNP) on PIAA Sec 6 form
- PIAA Section 6 must be dated on or after May 1, 2026 for the 2026-27 school year.

- Must be completed by an Authorized Medical Examiner (MD, DO, PAC, CRNP, or SNP) on PIAA Section 6 form and dated on or after May 1, 2026 for the 2026-27 school year.
- Print blank Sec 6 and take to physical exam for physician to complete, sign and date.
- Parent/Guardian must upload completed and signed form to Healthy Roster.
PIAA Section 7: Re-Certification by Parent/Guardian
- Required for Winter and Spring athletes if Section 6 is dated more than six weeks prior to start of sports season.
- Must be completed no earlier than 6 weeks prior to the start of the sports seasons.
- If any questions are answered "YES," PIAA Section 8: Re-Certification by Licensed Physician must be completed. If all questions are answered "NO," further action is not needed after both student and parent sign the form.
CDSD Certain Medical Conditions Medication Form (Not Required for all athletes)
- Complete this form electronically ONLY if your student has been diagnosed with one of the following medical conditions: (1) Asthma requiring an Inhaler (2) Severe Allergic Reaction requiring an EpiPen (3) Type 1 Diabetes and/or (4) Epilepsy.
- To access form: Go to Healthy Roster Documents. Click ADD. Select CDSD Certain Medical Conditions Form from the "Select Document Type" drop-down menu. Select "Fill Online.

Due Dates (2026-27) for Completion of Healthy Roster Documents
Concussion Management
Student-athletes who have been diagnosed with a concussion will be monitored daily by the CD Athletic Trainers and evaluated by the CD Team Physician. They will complete a "Symptom Score" sheet that includes 22 different signs and symptoms often experienced with concussions. Headache, dizziness, disruption of sleep, sensitivity to light and sound, as well as difficulty concentrating in school are among the most commonly recorded symptoms. School Accommodations will be provided after evaluation by CD's Team Physician if necessary.
Although every concussion is unique and may heal differently, the typical return-to-sport time is about a week to 10 days from the time of injury. Our standard return to play protocol involves the following steps, each performed progressively with approximately 24 hours between each step. If any concussion-like symptoms occur during a phase, it is immediately stopped and the Athletic Trainer will wait an appropriate amount of time before attempting this phase again.
Phase 1: Exertion (cardiovascular activity, such as bike, treadmill elliptical, in controlled environment)
Phase 2: No contact, sports-specific activities
Phase 3: Light contact, sports-specific activities/light practice (no live scrimmage)
Phase 4: Full contact, full practice, including live scrimmage
Phase 5: Return to game
Injury Management
- Immediate Evaluation: Athletic Trainers assess injuries on the field and provide initial first aid
- Return to Play (RTP): A mandatory written doctor's clearance note for any injury or illness seen by a physician is required before the athlete can return to play
- Concussion Protocol: Follows a 5-day, gradual return-to-play protocol supervised by ATs and Team Physician
- Documentation: Athletic Trainers keep detailed records of injuries, follow-up care, and parental communication
- Emergency Care: Athletic Trainers manage emergencies, contacting EMS when necessary
Responsibilities:
- Athletic Trainers: manage emergencies, contact EMS when necessary. Notify parents about athletes' injury via phone or email, offering home care instructions and coordination of follow-up care if needed. Will coordinate return to play with Team Physician, Parents and Coaches.
- Athletes: Report injuries or illness to the Coach and/or AT immediately. Athletes will not be allowed to return to play without a doctor's clearance note if seen by a medical professional outside of Penn State Health.
- Parents/Guardians: Notify Coach and Athletic Trainers if athlete is injured. Provide required doctor's note(s) to Athletic Trainers via Healthy Roster if athlete is seen by a healthcare provider for an injury or surgery.
- Coaches: Communicate with Athletic Trainers about known athlete injuries and illnesses for coordination of participation and safe return to play
- CD Team Physician: Will have final determination for return to play of all CD Rams athletes. Is available for injury evaluation weekly at CDHS and in PennState Health office by appointment

Lisa Wawrzyniak , MAT, LAT, ATC, started at Central Dauphin as an Athletic Trainer in January 2024. She completed both her undergraduate and graduate studies at the University of North Carolina-Chapel Hill, earning a Bachelor of Arts in Health and Physical Education and Master of Arts in Teaching in Health and Physical Education with specialization in Sports Medicine.

Athletic Trainers:
- Are highly qualified, multi-skilled health care professionals.
- Work under the supervision and in collaboration with a physician to optimize safe participation of athletes.
- Have at least a Bachelor's Degree in Athletic Training.

- Complete mandatory continuing education to keep skills current.

Athletic Trainers are available after school to provide the following services:
- Assessment and diagnosis of injuries and illnesses to create effective treatment plans for athletes.
- Management of emergency situations which may arise during athletic practices and events.
- First aid, treatment and coordination of follow-up care if needed.
- Notification of parents regarding athletic injuries via phone, email or Healthy Roster.
- Therapeutic Intervention and rehabilitation of injuries to restore function.
- Injury Prevention and Wellness Promotion focused on safety, risk reduction, and proper techniques to avoid injury.
